By clicking here you can access the Portal, as well as clicking on the “New Patient Portal” button at the top of our page.
At the moment, the new Portal will not have historical appointments or records, so we will be leaving up the previous Portal as well. If you would like to access it, please click here, or click on the “Previous Patient Portal” button at the top.

Dear Sunset Families,
We appreciate your attention to this important information!
On Tuesday May 23rd, we will be transitioning to a new Electronic Medical Records system. We look forward to this transition as it will allow our staff to better serve your family in the future. Due to this transition, there are several changes you will encounter or experience in the coming weeks and months. Please read carefully below to see how this change will impact your family, both at the time of your visits and outside of your visits.
Before your visits:
Sunset is excited about a new feature that is coming with this transition. Along with your confirmation emails/texts before your visit, you will now also receive a check-in link several days ahead of your visit.
This link will allow you to:
- Review your demographic information
- Review your insurance information
- Complete some or all of the forms our office will need
- Pay your copays
- See important information from Sunset that may impact your visit
During your visit:
We are working hard to ensure this is a seamless change for your family, however there may be times where we are a little slower in the next couple weeks to ensure we capture or update your information correctly in our system. We understand your time is valuable and we will be doing everything we can to ensure your visits to Sunset continue to be a positive experience.
Due to the system change, we will need to recollect certain information that you have likely already given us. We apologize in advance for the inconvenience, and ask that you please be prepared to potentially provide the following:
- We will need all families to put their card on file down again in our new system. For security purposes, card information is never transferred from system to system.
- Our new system contains a new patient portal (more information about this below). We may ask you to complete the portal access forms again at check in.
- We may need to collect a new copy of your insurance card if you have not uploaded that ahead of time.
- We may ask to verify more information than you are used to at check-in for your first couple visits on our new system.
- Over the next few weeks, requests for medical records, copies of statements and other information may take a little longer to produce. We will work our hardest to get that to you as soon as possible.
After/in between your visits:
After your visit, you will continue to receive a post visit survey, though it will look a little different. We appreciate your feedback as this helps us continue to improve the way we serve your family.
After or in between your visit, you will also be able to access your patient portal. If you do not have any upcoming visits and would like to set up portal access ahead of time, please contact our office to do so. You will continue to have portal access on our old system for the foreseeable future. We will notify families if/when that access will no longer be available.
New Patient Portal:
Once you log in, you will have a variety of self-service options to make accessing your child’s information more convenient for you.
Some of the features include:
- Access to your updated immunization records from home
- Access to your after visit summaries from each visit
- Access to patient education your provider or their clinical team uploads
- Access to your billing statements and balances, as well as your ability to pay remaining balances
- Access to review and update your demographics
- Ability to message different depts in our office at any time
- Access to lab results
- Access to referral status tracking
- Ability to submit prescription refills
- Upcoming appointments
- And more…
Please note:
Information for visits prior to 5/23/23 will continue to be in the old patient portal. The new portal will contain information for all visits starting on 5/23/23.
