To ensure the highest quality care is provided to your child, Sunset’s pediatricians only accept patients who plan to establish long term care with our clinic. Please first call our office to let us know you would like to join our practice. Our staff will collect your demographic information and provide you a login for our patient portal.
Before Your First Visit
To help things run smoothly, please complete these steps prior to coming in for your first visit.
Please contact your previous physician office to transfer any medical records including immunization records they have for your child to Sunset. Click here for a medical release form. This helps our providers know what medical care your child has received in the past and how to best take care of them moving forward.
Prior to your first visit, please download a copy of the new patient forms here and bring them with you to the first appointment. You will need to complete these forms at home prior to coming in so that you can focus completely on the appointment with the physician.
Please arrive several minutes before your scheduled appointment time to allow the office time to verify your information or collect any additional information we might still need.
Please bring with you: • A current form of picture identification • A list of current prescriptions your child is taking • Your current insurance card • A HSA, credit or debit card to be kept on file in accordance with our Card on File policy

Congratulations on the recent birth of your baby! Whether this is your first baby or you are a veteran parent, we are thrilled that you have chosen Sunset as your child’s medical home. To ensure the highest quality care is provided to your child, Sunset’s pediatricians only accept patients who plan to establish long term care with our clinic. Please call our office and let us know you would like to join the practice of one of our pediatricians. Our staff will be able to collect your demographic information and get your appointments set up.
Within the first 30 days of your baby’s birth
Please contact your insurance company and make sure that your newborn is added to your insurance plan. This important step helps to make sure that your child’s first few appointments will be covered in a timely manner.
